Support Coordination
OUR SERVICES
What support coordination means to us
At QLD Support Coordination, we believe everyone deserves support that feels clear, comfortable, and truly centred around their life. Support coordination is here to help you understand your NDIS plan, connect with the right services, and feel confident navigating everything at your own pace.
We’re here to walk beside you — not rush you, not overwhelm you — just support you in a way that makes sense for your goals and your everyday life.
How we support you
Support coordination is a capacity-building service, which simply means we help you:
Understand your NDIS plan without the jargon
Set up your supports in a way that genuinely works for you
Connect with providers you feel safe and comfortable with
Build the confidence to manage things more independently over time
Our aim is always to make the NDIS feel less stressful and more empowering.
Levels of support we offer
Support Connection
We help you link in with community, informal, and funded supports. This is all about building your foundations and confidence to coordinate things in the future.
Support Coordination
Here, you get more hands-on guidance. We help you understand your plan, organise your supports, troubleshoot issues, and make sure everything is running smoothly.
Specialist Support Coordination
For people with more complex needs, we provide specialised support to stabilise services, manage challenges, and ensure consistency and safety across all your supports.
Our role as your support coordinator
When you work with us, you can expect someone who will:
Listen first, act second
Explain things clearly and honestly
Help you connect with the right services
Keep an eye on your funding and supports
Encourage your independence and choice
Stand with you during tricky or stressful moments
You stay in control — we’re simply here to make the journey lighter.
Accessing support coordination
Support coordination is funded under the Capacity Building budget in your NDIS plan. If it’s included, you’re free to choose a provider who feels right for you.
If you ever need guidance, want to switch coordinators, or simply want a clearer understanding of your options, we’re always here to help.
Psychosocial Recovery Coach
OUR SERVICES
What is a psychosocial recovery coach?
A psychosocial recovery coach supports people living with mental health challenges to feel more confident, build daily living skills, and work towards goals that matter to them. The focus is on empowerment, stability, and creating practical strategies that help each person move forward at their own pace.
How can a recovery coach help?
A psychosocial recovery coach is trained to work alongside people with psychosocial disabilities, especially those supported by the NDIS. Their role is to make day-to-day life feel more manageable, strengthen mental health and wellbeing, and guide individuals through their recovery journey.
Many coaches also bring lived experience, which can make their support feel more relatable, grounded, and understanding.
What does a recovery coach do?
Your recovery coach may support you with:
Personalised guidance that fits your experiences, needs, and priorities
Goal setting that’s realistic, meaningful, and aligned with your recovery journey
Navigating the NDIS and community supports, including mental health services and local resources
Building confidence and independence, using practical strategies that work in everyday life
Why do people choose recovery coaching with us?
Support that feels tailored to where you’re at and what you want to work toward
A strength-based approach that focuses on your abilities and resilience
Consistent check-ins and ongoing guidance, helping you stay steady, supported, and encouraged throughout your recovery
We aim to make the journey lighter, clearer, and more empowering.
